Most of our programming and activities require registration.  Below you will find a list of registration forms, which should meet your needs.  If you are not finding the form you need please feel free to contact our General Programs Supervisor.

At this time, we are unable to offer on-line registration and/or payment.  However, you can download the appropriate forms and complete them.  Upon completion bring the form(s) with payment to the Avon Recreation Center to complete the registration process.
